Position Overview

As a Home Health Community Liaison, you will play a key role in expanding awareness of our services and helping connect patients with the care they deserve. This role blends relationship-building, strategic outreach, and compassionate communication with healthcare providers, facilities, and families across the Metro Detroit area.

Key Responsibilities

Build and maintain strong relationships with physicians, hospitals, assisted living, and skilled nursing facilities.

Identify new referral sources and implement effective outreach strategies.

Coordinate patient referrals and serve as the first point of contact for new partners and families.

Educate the community about available services and participate in discharge planning when needed.

Collaborate with healthcare teams to support patients and families in accessing needed resources.

Qualifications

Minimum 1 year of experience in home health or hospice sales/marketing.

Clinical background preferred.

Valid drivers license, reliable insured vehicle, and clean driving record.

Excellent communication, presentation, and interpersonal skills.

Compassionate, motivated, and team-oriented with a strong sense of accountability.
